At Care Pathway navigators we take care of the process for you. We complete applications for NDIS funding and then continue to work with you to put the correct support plan in place.​

If you have a support plan in place and your needs have changed CPN will review your current plan and make application for an upgrade

CPN take care of NDIS applications, we complete all paperwork and liaise with NDIS, Services Australia and work with local service providers to ensure your plan works for you.​
